​What does this change mean for you?


​Here are some important details about this transition:

  • Rest assured, there will be no interruptions to your care. You’ll continue to experience the same quality of care you’ve always had. 
  • With 4C Medical Group, you will enjoy additional benefits as a patient. In addition to providing telehealth visits, 4C offers acute and post-acute care and wellness services such as counseling, physical therapy, and nutrition counseling. 
  • We will continue to accept all insurances. However, you may receive a letter from your insurance company about our health services no longer being covered as in network. If you receive this letter, please contact us to verify your coverage and discuss the options available to continue seeing your preferred provider. 
  • Eventually, you’ll be asked to fill out new patient paperwork once our practice has transitioned to 4C’s electronic medical records.
